I need to geocode 12,000 addresses, so I enabled billing and set up credentials to use the Google Geocoding API. When I try to run the MMQGIS geocoder using my new API Key, I immediately get the message: Request Denied – 0 addresses geocoded. I have enabled the Google Maps Geocoding API as well as the Google Maps Javascript API.
What else do I need to do to get this working?
you should get an API key of your own, as @user30184 suggested.
without an API key, the plugin uses this key-less endpoint

This is only really intended for demonstration purposes.
As @mapperz points out, this is limited by IP address. See this answer for more details. You might be sharing this quota with others, which would explain the outages.
You should be able to paste your API key into the dialog. If you don't see it, upgrade MMQGIS to the latest version in the plugin manager.
From a quick look at the code, it looks as if one call is made per row , but these are spaced out to a maximum of 2 calls/sec to avoid problems with rate limiting.
Is there another way to geocode in QGIS?
with MMQGIS you can change the webservice to Nominatim .
No more need an API key, but please have a look to the policies

Figured it out and posting for those who encounter the same issue. I got the full error from Google:
REQUEST_DENIED Requests to this API must be over SSL. Load the API with "https://" instead of "http://".
To fix, change the URL in line 288 of mmqgis_library.py to begin with https.
