For some reason, when I removed and replaced toolbars in an .mxd document the Layout toolbar is greyed out.
I've tried opening/closing the document and ArcMap, in addition to re-adding the toolbar several times, and also tried going into Data Frame Properties and ensured that automatic radio button was checked.
Any suggestions?
In your template mxd, the inset data frame should be placed before the main data frame. When your users import the template, the inset will move to position 1, and the main data frame will be in position 2.
When applying the template to an mxd with a single data frame, the default layers data frame will occupy the inset position.
I had a user struggling with this earlier. When you switched to layout view, you saw a very tiny small blue square and that was it. By closing the map and changing the printer/page layout to 'scale to fit' while we were still in data view and then opening layout view it worked.
Next step was deleting normal.mxt and replacing.

Make sure you're using the Layout View.
Of the 3 little buttons under the map data window, the left is data (map) view, the middle is layout view - this is when the layout toolbar is active, the right is refresh.
