I want to create a clean user interface for my students to use QGIS. I tried to remove the menu bar using the customization in setting but I am not able to remove everything, e.g. Database, plugin, etc. Is there a way to remove them?

The easiest solution is to start QGIS with the --nocustomization command line option and fix things from the GUI.

Starting from QGIS 2.0
In case you have still access to the python console (In the Plugins menu): Enter the following command which will disable the customization:

QSettings().setValue( '/UI/Customization/enabled', False )
If not, you're going to have to fix this in your registry by setting the key "enabled" to false: (Use regedit to get access)

You can add a custom menu to the QGIS GUI this way:
self.menu = QMenu( "&My tools", self.iface.mainWindow().menuBar() )
actions = self.iface.mainWindow().menuBar().actions()
lastAction = actions[-1]
self.iface.mainWindow().menuBar().insertMenu( lastAction, self.menu )
As you can see in the code snippet above, you are adding a menu to the second to last position of the menu bar, right before the Help menu.
Then, you can add an action to your newly added menu this way:
self.menu.addAction( self.action )
You may already know, but just to make it clear, such GUI configuration should normally be located in the initGui() method of your plugin.

If you go the the Settings > Customisation menu options, it gives you a dialog that allows you to turn off whole toolbars and also remove menus and docks. You can also remove individual buttons or menu items too rather than the whole toolbar or menu.
You can also save these settings from this panel to an ini file, and then open an ini file on another machine.
