For orthorectification:
In GRASS see i.ortho.photo.
In OSSIM, see OSSIMOrthos.pdf
For georeferencing:
In QGIS, use the Georeferencer plugin, p 223 of the user guide.
There is even an online tool for georeferencing at http://www.georeferencer.org/
