The undertilde package provides extensible tilde accents under elements in math mode via \utilde{<symbols>}. Try, for example:
\documentclass{minimal}
\usepackage{undertilde}
\begin{document}
This is $\frac{\utilde{ab}}{c} \neq \frac{ab}{c}$ and so on.
\end{document}
Alternatively, as @egreg suggests, the accents package provides \underaccent[<accent>]{<symbol>}. Try, for example:
\documentclass{minimal}
\usepackage{accents}
\begin{document}
This is $\underaccent{\tilde}{\mathcal{A}}$ and so on.
\end{document}
See page 2 of the package documentation in order to extend this using \widetilde rather than \tilde. It depends on your preference.
I'm afraid you hit on something that can't be easily solved. Here for comparison all possible solutions for making math stuff bold and then a handmade bold tilde T:

% not bold

% not bold enough either

% maybe even crayons would be easier and prettier but well ...

Using .8em in the raisebox will be about the same height as the other tildes but I personally liked it better this way. I'm not saying it's pretty and I'm not saying it's easy :/

\tilde{\jmath}, but one may argue that thejcharacter is not really nice then.
