[Tex/LaTex] Footnote in table appears on wrong page

I have a tabular in a floating table environment and want to place a footnote in that table, whereas the footnotetext should appear at the bottom of the page where the table is on. I tried the following:

\footnotemark inside the table and \footnotetext{...} after \end{table}
\tablefootnote with the tablefootnote package
Surround the table environment by \begin{savenotes} and \begin{savenotes} with the footnote package

All these solutions produced a footnotemark and corresponding text, however, none of the solutions placed the text of the footnote on the appropriate page.

EDIT: For smaller tables (e.g. only the first 4 rows in my MWN), the table appears beneath the footnote which seems even worse…

This problem has no standard solution. Footnotes in floating environments in general belong inside the floating environment. Some ideas how to achieve this are:

enclose the inside of the table in minipage:

\begin{minipage}

C\footnotemark & D\\\hrule
\end{tabular}
\footnotetext{This is a `C', you see?}
\end{minipage}

not let the table float at all, using \captionof from caption package
use threeparttable package

You should never have a blank line before a display equation it creates an anomolous white line (not vertical space) that is not wanted above the display and is not discarded at a page break.

Although even without that the specified space does not allow a footnote on page 1. there are two problems, to make it fit you have to take a single line of the paragraph over a page, and normally LaTeX tries to avoid doing this (\widowpenalty) secondly as there is not enough stretch space on the page to make up for the difference in line heights.

If you make TeX prefer rather than avoid widow lines then it fits (I don't recommend doing this, but to demonstrate the effect)

The document produces the warning

Package multicol Warning: I moved some lines to the next page.
(multicol)                Footnotes on page 1 might be wrong on input line 26.

multicol can detect that it has happened but can not go back to fix it.

You can force a page break with the correct footnotes:
